When to play Truth or Dare
Truth or Dare is perfect when you want action: people doing silly things, confessing in public, and everyone watching. It needs no setup at all and works with any group that is up for it.
When to play Paranoia
Paranoia is for when you want suspicion instead of spectacle. Nobody has to perform — one name, said out loud, is enough to set the whole room off. It is easier on shy players, more dramatic for big groups, and it works online.

Can you combine them?
Plenty of groups do: whoever gets named in Paranoia has to take a truth or a dare to hear the question. In the Paranoia app, forfeits and consequences are built in, so you do not have to invent them.
